What a modern dental implant is, and how to choose between procedures

The implant procedure has evolved far past the single-tooth-replacement framing most patients arrive with.

The titanium root and why osseointegration matters

A dental implant is a threaded titanium fixture placed into the alveolar bone where a tooth root used to sit. Over the following twelve to twenty-four weeks, osteoblasts deposit new bone directly onto the textured implant surface in a process called osseointegration, a phenomenon first documented in Per-Ingvar BrΓ₯nemark's research in the 1960s and now supported by more than five decades of clinical follow-up. Once integrated, the implant functions as a biologic root: it transmits chewing forces into the jaw, prevents the bone resorption that follows tooth loss, and supports a crown, bridge, or full-arch prosthesis with stability that adhesive-retained dentures cannot match.

Choosing among single tooth, All-on-4, and zygomatic protocols

The right procedure is determined by what is missing and what bone remains. A single-tooth implant replaces an isolated lost tooth without altering the adjacent teeth, which is why it is preferred over a traditional three-unit bridge whenever bone volume permits. The All-on-4 protocol replaces a complete arch with four implants and a fixed prosthesis; the two posterior fixtures are tilted to anchor in denser bone and avoid the maxillary sinus. For severe upper-jaw bone loss, typically patients who have worn an upper denture for ten or more years, zygomatic implants anchored into the cheekbone are often the only same-day fixed solution. Apex performs all three protocols in-house, which is uncommon outside major academic centers.

How candidacy is actually determined

Every consultation at Apex begins with a CBCT (cone-beam computed tomography) scan, which renders a sub-millimeter-resolution three-dimensional reconstruction of the jaw, sinuses, and inferior alveolar nerve. This single image determines bone height, density, and the safe trajectory for every potential implant site, and it is the basis on which we either confirm candidacy, recommend an adjunct procedure such as bone grafting or sinus lift, or, in a meaningful number of cases, redirect the patient toward zygomatic implants that other practices would not have offered. The scan is included with the consultation at no cost, and the written treatment plan reflects what the imaging shows rather than what the patient was told to expect.
